We are so happy you are considering Greenwood-Molina Nursery School as your childcare provider. To start the enrollment process, please download the Admissions Form, and either the Child Development Form for children 18mnths – 5yrs not in Kinder, or the School age Questionnaire for Kinder-12yrs.
Additional documents required:
Your child’s current immunization record, along with one of the following options is required:
- Have a health-care professional sign and date the back of the admission form stating that your child has been examined.
- A signed and dated copy of your child’s most current physical
- Parent acknowledgement stating child has been exanined by a healthcare professional and signed statement will be submitted within 12 months of admission
- A signed and dated affidavit stating why your child’s examination by a healthcare professional would conflict with the tenants and practices of a recognized religious organization.
Please note that the state requires that all children ages 4 years of age must have a vision and hearing screening unless the child has had a screening in a Headstart or Pre-Kinder program. The screening is required during the first four months of enrollment.
If you will be enrolling your child under Private Pay/Tuition Assistance, we also require that the originals of the following documents be brought to our office upon submitting your admissions form:
- Parent enrolling child will need to provide current ID or driver’s license.
- Original state issued birth certificate for child being enrolled. Parent’s name must match ID/driver’s license
- Child’s social security card
Copies of these documents will be made at the center.
Once admissions form, child development (for children `8 months to 5 years not yet in kinder) or school age questionnaire (for children in kinder to 12 years of age) and other legal documents are submitted to our office, a 45 minute parent orientation will be scheduled. This will be done over the phone. We will provide you with your copy of our policies, as well as additional paperwork that will need to be completed and turned in on your child’s first day. Please arrive early on your child’s first day so that the additional paperwork can be reviewed, and to answer any questions you may have.
